Telemedicine Visits

Telemedicine visits are available for Arizona residents and established patients traveling out of state. These are billed the same as in-person visits whether the format is secure video within the patient portal or telephone.

Health Insurance & Payment

If you need to cancel or reschedule your appointment please do so at least 24 business hours before your scheduled appointment. Since your appointment time is reserved exclusively for you, a nominal $50 fee will be billed to your account when no notice is provided.

Our goal is to offer premium care and service specific to each patient’s needs without having a third party payer dictate a patient’s diagnostic or treatment plan. Hence, we are not contracted with any insurance carriers and are considered “out of network”. Payment is due at the time of service via cash, credit, debit or HSA/FSA cards. We are happy to provide insurance coded receipts if you would like to submit a claim to your insurance company. Many patients receive credits towards their deductibles or partial reimbursements.

Cancellation policy

New patients should complete all consent forms and health questionnaires prior to your initial visit. These forms are sent to you via email and can be completed online. Click “save” and “submit” to ensure they are uploaded into your portal. This will take about 15 minutes.

Please arrive on time so that you can benefit from the full amount of time reserved for you.

Arrival

Please update your medications and supplements in the patient portal, complete with names and strengths. Please upload any relevant and recent lab results, medical records, or imaging reports.
